Mind Possible Affiliate Application
Step 1:Please download, fill out (hand-written is fine), and return a digital copy of the application form (see below). The bottom of the form states where to e-mail the form. There is no fee for applying, and no fee after your application is accepted.
Before you apply, please know the most common reason for ending our affiliate agreement is because of lack of sales. Please be sure you have sufficient traffic to your site before asking to become an affiliate.

Step 2: Accepted applicants will be notified of their acceptance within 7 days of applying. If you don't receive a response within 7 days, you can assume your application was not accepted. If your application is accepted, you will be sent a W-9 (U.S.) or W-8BEN/W-8EN-E (non-U.S.) tax form to complete. Once you return it, you'll be sent your affiliate links and marketing images.
